Police

The Dunn Police Department cares about its citizen’s safety and well-being and in particular, our senior citizens.  Should a medical emergency happen, it is vitally important to be able to communicate your medical conditions, needs, medications, allergies, hospital preference and so on to the responding emergency personnel.  But what if you are unable to communicate your needs or wants at a time when it is most important?  Having a prepared Emergency Medical Information Packet posted on your refrigerator door could be a life saver.  The packet consists of a large envelope that is boldly identified so responding personnel can quickly locate the vital information to assist in your care.  Inside the envelope is a medical information questionnaire containing pertinent information about your medical history, medications, Doctors, hospital, emergency contacts and much more.  You should include a recent photograph to assure your identity to the responders.  You can download a copy of the medical information form to fill it out.  You can also request an envelope like this one on our contact page, or come by the police department and pick one up Monday-Friday during business hours. This is a free service provided by the police department.
